If you were unable to find the provider you were looking for, please check back or reach out to our Customer Service department for more details. Please note: We are working with additional Durable Medical Equipment (DME) vendors, Byram Healthcare and Apria Healthcare to provide more in network DME options. In addition, we have a national contract with Labcorp and Quest, which may not be reflected in our printed directory. We are working to obtain facility information for our directories at this time.

Medicare-covered chiropractic and Medicare-covered acupuncture services are administered through our partner, American Specialty Health (ASH). Those providers are not listed in our Provider Directory. To locate an ASH provider for these Medicare-covered services, please click here.

Network Coverage Rules

You can choose to receive care from out-of-network providers. Our plan will cover services from either in-network or out-of-network providers, as long as the services are covered benefits and medically necessary. However, if you use an out-of-network provider, your share of the costs for your covered services may be higher.
